Does one film with a character, workflow and story world require four separate Index records?

No. Define the asset you actually wish to describe and offer: AIF for a film, WF for a workflow, IP for independently usable intellectual property and WLD for a story world. Create related records when they have independent presentation, versioning or licensing needs. A finished-film record may be enough initially. Avoid duplicate or contradictory rights descriptions.
